We would first make sure you have insulation in the wall or ceiling cavities and if not then we would blow in insulation. Then add a layer of QNRLV-100 loaded vinyl barrier, QRSIC-1 isolation clips with furring channel and one or two 5/8" drywall. This takes up less than three inches of interior space.
Cost depends on access and details but usually runs around $50.00 per Square Foot.
Here is some product information for you.

There are other effective applications that cost a bit less but have a lower STC rating. Below is a product that would just go over existing drywall.
Green Glue is our newest product. It works very well for the cost and may be installed by contractors. It reduces noise transmission through walls and ceilings by ³damping² the vibrations caused by ambient and structure borne noise somewhat like putting a rubber mat over a drum head.
